SHS Policies, Procedures, and Practices Suggestions & Requests Form
Initial Policies (January 10, 2012)
The primary purpose of the School of Health Sciences Fitness Center is to provide a space for fitness related educational, research and clinical activity. Use by SHS Sciences faculty, staff & students is welcome during open times and when the Fitness Center is not being used for other purposes.
Priority Usage

3. General SHS Faculty, Staff, Students & Affiliated Personnel Use
o Affiliated Personnel:
▪ Family members of SHS faculty and staff who are 18 years of age or older and have a valid Phoenix Card.
▪ Undergraduate faculty, staff & students using the facility for research or clinical purposes.
Times: Open 6 am – 9 pm (including weekends)
● Scheduled open use times *(open to SHS faculty, staff & students):
o 6 am-9 am
o noon-1 pm
o 5 pm- 9 pm
o Other times, not scheduled, are open
*There may be times where faculty or staff need to schedule an educational, research or clinical activity during the “Scheduled open use times”
● Scheduled educational, research, or clinical time (generally all times except “Scheduled Open Use Times”)
o The Fitness Center can be scheduled ahead of time by faculty or staff for educational, clinical and research use. Generally, the space should be scheduled at least 1 week in advance; however, in the event of unanticipated educational needs (e.g., class schedule changes) the educational use of the fitness center takes priority over scheduled research or open personal use.
o Unscheduled times are considered “Open Use” times but may be interrupted for educational purpose per above.
Usage Policies (adapted from the Campus Recreation Fitness Center Policies)
● No one under the age of 18 may use the facility unless they are participating in a supervised educational, research or clinical activity.
● No one may use the fitness center outside of the regularly scheduled open hours.
● No one should use the fitness center when alone in the building.
● Proper attire required: tee shirt covering midriff, athletic shorts or pants, and sneakers.
● No jeans, open-toed or open-heeled shoes.
● No food, gum, or tobacco products permitted.
● 30-minute time limit on all cardio equipment.
● No cell phone use in fitness center.
● Do not drop weights.
● Wipe down all equipment and mats.
● Rack all weights after use.
● Report injury or equipment issues immediately to Ms. Reidin the Department of Physical Therapy Education. If Ms. Reidis not immediately available, then report issues to a DPTE faculty member.
● SHS faculty and staff are authorized to remove individuals from the SHS fitness center if they are deemed unruly or uncooperative

Serious medical emergencies CALL 911
Remember...Stay calm. Help others remain calm. When you are reporting an emergency situation give concise, accurate answers to the 911 operator. Do not hang up until told to do so.
The Campus Safety and Police phone number is 278-5555—on campus ext. 5555.
Francis Center AED (Automated External Defibrillator) Location: Mounted on the wall next to the student lounge door (Room 102; within the commons area).
A. In case of minor injuries, you can contact Campus Police at ext. 5555. An officer will respond to assess the situation and render assistance. They do not provide transport to doctors office visits, pharmacies, or out-patient care services.
B. Things to remember:
1. Keep the victim still and comfortable.
2. Do not move any injured person unless there is an immediate threat to their life - then move the victim only as far as necessary.
3. Ask the victim “Are you OK?” or “What is wrong?”
4. Check breathing and give cardio-pulmonary resuscitation (CPR) if necessary.
5. Control serious bleeding by direct pressure on the wound.
6. Continue to assist the victim until help arrives.
7. Look for a Medic-Alert or other medical ID tag.
8. Question witnesses.
9. Give all information to the 9-1-1 operator.
10. Upon arrival the FD/EMS person will be the OIC (officer-in-charge).
C. Medical assistance should only be administered by a certified provider.

Incident Report
Emergency Situations: The top priority is taking care of the immediate needs of the injured person(s) following the emergency procedures described above. Formally reporting the incident to the DPTE should occur immediately after the emergency situation has resolved.
Any accident, incident, or "near miss," no matter how slight the injury or damage, must be reported to the DPTE administrative assistant. A form will be completed for each occurrence. The DPTE administrative assistant will report the incident to the DPTE Chair who is responsible for taking appropriate follow-up action
