Identification (ID) Badges
Phoenix and picture ID badges will be taken on or before the 1st week of Module I. Each student will also be provided with a “First Name” badge in large text that attaches to the student ID to make the name visually accessible to patients, faculty, and staff. Students are required to wear their IDs when interacting with clients-to-class and otherwise upon request. ID badges must be worn during clinical education experiences unless the facility requires a particular badge. Any costs for facility-specific ID badges will be borne by that clinical site or by the student.
